PCP Interview Call + Enrollment Call Questionnaire

After you confirm your phone call, ask your PCPs if they wouldn't mind responding to a few questions prior to your call so you can make the most of it:

"Hi [name],

We're confirmed for our call on [date/time]. I'll be calling promptly at that time. Before then, would you mind answering the following questions so we make the most of our time together? Thank you SO much!

1. What's holding you back from [problem they need help with e.g. feeling in control and organized as a new mom]? What hurdles are you running into?Tell me everything. :)
2. What have you tried so far to help you [become a more organized and confident new mom]? What results have you achieved so far?
3. Take a moment and daydream for a minute... 1 year from today, what does your life look like? What's your ideal?

When to use the Enrollment Call ONLY Questionnaire

• For PCPs you want to enroll, who didn't enroll during your interview call. 
• When you've reached Module 6 and send your "Work With Me Email"  
• Customize the questions to match your niche and PCP.
• Optional: Include "course starts in a few weeks or X date" to the "selectivity"
  question to create urgency for your PCP to enroll on the phone.
